Objective: To ensure equal housing under the CILA Program for any prospective resident regardless of race, color, sex, religion, nation origin, or disability.
Admission Criteria:
1) The Applicant must be (18) years or older
2) They must have a Developmental Disability
3) Must be in need of array of services and supervised living arrangement
4) Must give an informed consent to participate in the CILA by the individual or guardian. This must be documented in the individual record
5) Must agree to participate in the development and implementation of the ISP which shall be documented by the indivudual or guardian signature either on the ISP or a note explaining why there isn't a signature.
6) Must be approved under the guidelines from a DDD approved PAS Agency
Documentation Requirement:
1) Pre-Assessment Screening (PAS) Referral
2) Intake Summary Form
3) Proof of Medicaid Waiver eligibility
4) An Initial Assessment
Medial examination within (12) months preceding admission
Dental examination completed within (12) months of admission
A psycho-social assessment
An assessment with form DMHDD-1215, SLOF, ICAP or an SIB
An educational and/or vocational assessment
Communication screening that includes vision, hearing, speech, language and sign language
Screening by the Physical Therapist, Occupational Therapist and Activity Therapist that may be required based on the individual disability
Implementation of a Service Plan within (30) days of admission
